Med Reconciliation Pharmacist Position Overview

 

The Pharmacist supports pharmacy operations by inputting and dispensing medications as prescribed and by maintaining compliance with all state, federal and other applicable regulatory agencies. Oversees the Medication Reconciliation pharmacy team at Woodmark of New York.

Shift: Variable schedule, covering morning and evening shifts

Responsibilities:

Med Reconciliation Pharmacist Essential Job Functions

  • Oversees and supervises the Medication Reconciliation Pharmacy team.
  • Complies with all regulations, directives and works only within the boundaries of the pharmacist’s scope of practice.
  • Receives and inputs medication orders into computer via fax, electronic or phone from facilities.
  • Demonstrates a complete working knowledge of the pharmacy software package and facility EMAR system.
  • Provides drug information in an easily understood manner as requested by facility staff.
  • Researches and clarifies medication orders that are incomplete, unclear, or with clinically significant issues.
  • Professionally and proactively interacts with consultant Pharmacist, Medical Director’s, and other Healthcare Practitioners.
  • Attends staff meetings and required or preferred in-service training.
  • Travels to facilities for meeting, delivery, or inspections as required.
  • Maintains access to local RHIO, and hospital EMR systems as appropriate.
  • Exhibits the highest level of customer service towards staff, residents, families, and visitors.
  • Arrives to work on time, regularly, and works as scheduled.
  • Supports and abides by Woodmark’s Mission, Vision, and Values.
  • Abides by Woodmark’s businesses code of conduct, compliance, and HIPAA policies.
  • Performs other duties as assigned by supervisor, management staff.
Qualifications:

Med Reconciliation Pharmacist Educational Requirements and Qualifications

  • Credentials as a Pharmacist with a current state license in good standing
  • Residency trained – PGY1 Residency is required
  • Current, valid driver license in good standing
  • Long Term pharmacy experience preferred
  • Hospital experience required
  • Board certification in geriatrics or other field

  • Excellent technical skill, knowledge, and usage of specialized equipment in accordance with pharmacy/state regulations

  • Strong sense of professionalism and accountability
  • Self-directed and able to work autonomously as well as in a team-focused environment
  • Maintains highest level of confidentially and privacy handling sensitive information

This position requires regular interaction with residents, coworkers, visitors, and/or supervisors.
